Mako RPS (Robotic Power System) is a robotically enabled handpiece designed to give surgeons an intuitive, controlled cutting experience with robotic precision and a familiar handheld experience. The Mako RPS Total Knee workflow does not require preoperative imaging and uses intraoperative registration and real-time guidance technology to support implant planning, alignment assessment and bone resection throughout the procedure. After tracker fixation and anatomy registration, the system enables the surgeon to evaluate alignment, visualize soft tissue balance data and adjust implant positioning before performing guided resections. Following bone preparation, trial and final alignment analyses help the surgeon confirm implant positioning and overall knee balance prior to final implantation.

Step 1: Anatomy registration  

  • The surgeon completes bone registration on the femur and tibia by collecting points on the surface of the patient's bone. 
  • During anatomy registration the surgeon selects the tibial boundary point. This registration point is used to generate a plane that deactivates resection capabilities if crossed, helping prevent cutting beyond this surgeon defined boundary.

Step 2: Pose captures and implant planning    

  • Capture a flexion and extension pose in the desired limb alignment and laxity that they would like to achieve with their final implants.
  • Adjust and plan for the Triathlon Total Knee system. 
  • Both "gap balancing" and "measured resection" workflows are available.

Step 3: Bone resection

  • Mako RPS’s patented active adjustment technology responds to surgeons’ hand movements, guiding the handpiece to stay aligned with the surgical plan.  
  • Mako RPS integrates boundary control on the tibial cut: once the handpiece reaches the posterior protection boundary as defined by the surgeon’s selected landmark, resection capabilities are disabled.
