DESCRIPTION OF THE TRANSFERS

This Appendix forms part of the Data Processing Agreement and, if they are applicable, the Standard Contractual Clauses.

The Member States may complete or specify, according to their national procedures, any additional necessary information to be contained in this Appendix

Data exporter
The data exporter is the Customer as specified by the Agreement

Data importer
The data importer is Stryker as specified by the Agreement

Data subjects
The Personal Data transferred concern the following categories of Data Subjects:
Patients of the data exporter,
Employees of the data exporter
Third party vendors of the data exporter

Categories of data
The Personal Data transferred concern the following categories of data:
Contact information (incl. name, e-mail address, work extension number of customer personnel)
Customer traffic Data (as defined in Art. 2 lit (b) of Directive 2002/58/EC as amended by Directive 2006/24/EC
and Directive 2009/136/EC) and GDPR and billing data related thereto.
Personal Data contained in any content that is managed on behalf of the data exporter

Special categories of data (if appropriate)
Patient medical information

Processing operations
The Personal Data transferred will be subject to the following basic Processing activities:

Benchmarking of Personal Data to guide critical decision making.
Potential anonymization of Personal Data.
